Immediate Dental Implantation
The global practice of immediate dental implantation is now being successfully applied in Kyiv, with immediate implantation demonstrating excellent results and offering numerous advantages. It is a rare case where a technology saves both time and the body’s resources.
Patients of many clinics in Kyiv and beyond say that due to their busy schedules, they hesitate to undergo lengthy dental procedures. The logic behind such decisions is understandable but flawed: the longer a problem is ignored, the more time, money and effort will be needed to solve it. The good news is that many clinics in Kyiv, including ours, quite successfully use techniques that save time for both the patient and the doctor. Immediate dental implantation is a prime example of this.

Tooth extraction always involves bone trauma. If an implant is not placed at that very moment, healing will occur. Then, after healing, the doctor will need to inflict a new controlled trauma to the bone during implant placement.
At “Osadchiy’s Dentistry,” we believe it is better to overlay trauma upon trauma. This does not mean that an immediate implant will deepen or worsen the trauma. Human physiology is designed so that the body barely notices such a procedure, especially when non-aggressive implants are used. Immediate dental implantation does not affect the physiological process of bone healing.
Quite often, patients at various clinics in Ukraine make decisions about dental services based on price. However, the cost of immediate dental implantation in this case is not a determining factor, since it does not exceed the cost of delayed implantation.
Of course, immediate implantation has its pros and cons. By “cons” we mean not the drawbacks of the technology, but rather the impossibility of applying it in certain cases.
Tooth extraction with immediate implantation is contraindicated in cases of:
- traumatic tooth extraction;
- extraction of a tooth with a cyst;
- extraction of a tooth from severely damaged bone.
The key contraindication that makes immediate implant placement impossible is cysts or cystogranulomas of significant size. During the extraction of teeth whose roots are affected by cysts, what forms is not just a physiological socket, but a hole of very large diameter and depth. As a rule, in such cases the bone is also affected, so there is simply not enough bone material for the implant, and it is necessary to wait for recovery.

Tooth extraction with immediate implantation has the following advantages:
- minimisation of bone trauma;
- faster rehabilitation;
- fewer doctor visits;
- less anaesthesia required.
Immediate implant placement is the optimal and correct decision, which we insist on in cases where there are no contraindications.
If at the current moment the patient is not mentally ready for implantation or the decisive factor turns out to be the price, immediate implantation will not be performed. Many patients in such cases decide to postpone the extraction of the damaged tooth. However, this is wrong – even if the affected tooth does not hurt or cause discomfort, it must be extracted, because otherwise it will continue to destroy the bone, and further prosthetics will be greatly complicated.

Immediate implantation of front teeth and molars is no different. Immediate implantation of a front tooth is associated with the aesthetic appearance of the smile, so the problem must be resolved quickly. Waiting for healing when we can place an implant immediately makes no sense.
Another question is: should a one-stage implantation be performed in this case? Immediate implantation with instant loading is not recommended; we prefer safer and less risky techniques. That is, first the implant (artificial tooth root) is placed, and after the bone heals around the implant (21–60 days), an abutment and crown are placed on it. During the healing period, we provide the patient with a temporary structure that serves an exclusively aesthetic function.
When all teeth need to be extracted, immediate implantation is indicated. Not performing it immediately only prolongs the rehabilitation process and brings no benefit. The standard recommendation is: one extracted tooth – one implant. However, for cost-saving purposes, 4 to 10 implants can be placed on one jaw.
Immediate implantation of all teeth at our clinic is also performed in two stages. In the first stage (surgical), the teeth are extracted and implants are placed; if necessary, the patient receives temporary prosthetics. In the second stage (prosthetic), dental crowns and/or bridges are placed. Our clinic’s philosophy is to achieve a long-term result without risks. And one-stage implantation always carries risks, especially when it comes to implantation on an entire jaw.
If a patient needs immediate dental implantation, the price is formed according to a very simple principle: the cost of extraction plus the cost of implantation. The patient incurs no additional expenses; moreover, the total cost may even be slightly lower, as less anaesthesia will be used. Immediate and delayed dental implantation cost the same. You cannot wait too long after tooth extraction: the dental row may shift or the bone may decrease in volume. Bone healing from a blood clot to full-fledged cancellous bone takes from six months to a year. However, the bone element forms within 21 days. Therefore, on average, delayed prosthetics should be performed after day 21, but no later than two months after tooth extraction.
An implant can last a lifetime. The success of implantation depends on two aspects: whether the implant has integrated and how it is maintained. If everything has been done correctly and techniques to reduce the risk of complications have been used, the implant can last a lifetime. However, it is important to remember that everyone needs regular check-ups with a doctor and hygiene monitoring, whether it is their own teeth or implants.
The standard number of implants for an edentulous jaw is 6, 8 or 10 implants on the upper and lower jaws. However, the minimum acceptable number is 4 implants on the upper jaw and 4 on the lower jaw. They will serve as supports that hold the overall structure.
