Surgical dentistry is a vast and quite complex field of dentistry. Complex tooth extraction in surgical dentistry is associated with the removal of teeth of the 2nd or 3rd difficulty level. The 2nd difficulty level implies that the tooth does not wobble but may be very fragile or fused with the bone tissue. As a rule, when extracting teeth of the 2nd difficulty level, the tooth is cut and removed in parts. It is believed that tooth-preserving operations are an alternative to tooth extraction and allow the patient’s own teeth to be used for some additional time. However, the effectiveness and success rate of tooth-preserving operations are extremely low. At “Dr. Osadchiy’s Dental Clinic,” we very rarely recommend tooth-preserving operations to our patients.
Hemisection — this surgical operation in dentistry is necessary to preserve a tooth, one of whose roots is destroyed. After hemisection, a healthy tooth root remains, on which a crown is placed.
Replantation — to avoid tooth extraction, surgical dentistry used the operation of transplanting the patient’s own tooth from one socket to another. Previously, replantation could also be performed to extract the patient’s own tooth, carry out treatment, and subsequently place the tooth back into the same socket. This technique is outdated, and its low success rate speaks for itself: only about 5% of cases are successful. The need for replantation operations disappeared with the advent and spread of implants.
Root tip resection — this type of operation is usually performed in the presence of neoplasms on the tooth root and involves the removal of the neoplasm and cutting off the tip of the tooth root. It is most often performed on the frontal group of teeth, as their roots are easier to access.

It is important to note that in a number of clinics in Kyiv, surgical dentistry also includes sinus lift services.
A sinus lift is an operation to increase the volume of bone tissue on the upper jaw in order to obtain a greater bone supply for implant placement.
From the perspective of medicine and law, the performance of sinus lift operations by a dental surgeon is questionable, since during this procedure, the doctor goes beyond the oral cavity and penetrates the nasal area. Such operations should be performed not by a dental surgeon but by a maxillofacial surgeon.

Root tip resection is associated with several significant risks, the main ones being: re-infection, weakening, and increased fragility of the tooth root. This complicates the treatment prognosis. In addition, in some cases, the inflammatory process may be too extensive or deep, making apicoectomy impractical.
Yes, they can, and in some cases it is even necessary: for example, before orthodontic treatment or if the wisdom teeth are causing discomfort, pain, or provoke odontogenic sinusitis.
Even when extracting teeth of the 1st difficulty level, the doctor must be confident in the clinical picture. For this, they need at least an orthopantomogram. If something seems questionable to the doctor on it, a CT scan will be needed. An orthopantomogram can be done at our clinic.
A “dental abscess” is an inflammation with the formation of pus. In medicine, the rule applies: “If there is pus — open it.” Thus, in case of an abscess, you should consult a dental surgeon or a maxillofacial surgeon.
