Missing teeth, inability to use a removable denture
Critical vertical and horizontal atrophy of the alveolar ridge of the lower jaw. Complete edentulism of the lower jaw
Placement of 4 short i-system implants with bilateral distalization of the inferior alveolar nerve and prosthetics

A woman came to our clinic on the recommendation of a doctor from another dental clinic (as they were not prepared for such a complex case). Treatment of such a patient needs to be performed in a specially equipped dental surgery.
The patient was diagnosed with vertical and horizontal atrophy of the first degree of the alveolar ridge of the lower jaw. With this diagnosis, the use of any screw implants is impossible.
Previously, the patient had been offered a complex reconstructive surgery involving bone augmentation. The rehabilitation period after such surgery, in case of complete success, would have been approximately 2 years. The patient refused this treatment plan and came to our clinic.
Alternative treatment in the form of a complete removable denture was also impossible, since the vestibule (anatomical structure) of the oral cavity was practically absent (there was nothing to hold the denture on).
A surgical dentistry consultation was conducted to establish the diagnosis and develop a treatment plan.
The placement of 4 implants of the i-system plateau system was proposed (as the only possible option for implantation without bone augmentation) with subsequent fixed prosthetics on them.
A surgical plan was developed based on CT scan results.
Bone grafting was performed in the area of destroyed bone with distalization of the inferior alveolar nerve on both sides of the jaw using a piezotome.
Distalization is a complex technique where the nerve is moved aside to allow unobstructed placement of a dental implant.
After that, 4 short implants of the i-system plateau system were placed.
Recovery period: 2 months.
Two months after dental implantation, complete prosthetics of the lower jaw teeth with zirconium oxide crowns was performed.
As a result, the patient received functional and aesthetically attractive teeth.
