Basal implantation
Basal dental implantation is an implantation method built on the use of special basal implants that can be placed into bone with low density.
The technique got its name because the implant is placed in the basal part of the jawbone, which is an extension of the cancellous section. Thus, when a patient has insufficient bone availability or an atrophied jaw, some doctors recommend using the basal dental implantation method.
Basal implants come in various configurations, but their predominant characteristic is their resemblance to a self-tapping screw. Such implants are very aggressive – they literally cut into the bone.
Basal dental implantation in Kyiv has become a fairly popular procedure, but at our clinic we do not recommend it to patients and always urge a sober attitude toward risks. We believe that the disadvantages of basal dental implantation and the risks it carries are incomparable with its effectiveness. In nearly 37% of patients, basal implants integrate with immediate loading.
Disadvantages of basal dental implantation
Basal dental implantation in Ukraine is performed with immediate loading in most cases. Often, the possibility of obtaining quick results is the key factor for patients when making decisions. However, we do not recommend basal dental implantation, especially with immediate loading, for a number of reasons.
Disadvantages of basal implantation:
- poor integration of basal implants. To fix the implant in hard bone, it is necessary to pass through cancellous tissue, and it is precisely in this area that the implant fails to integrate;
- traumatism. Any implantation involves controlled trauma, but with basal implants, damage occurs to the deep layers of the jawbone;
- prolonged rehabilitation. Basal dental implantation in Kyiv is positioned as a procedure for quick tooth replacement. However, doctors do not always warn about the long recovery period: swelling and postoperative pain will accompany the patient for a long time;
- implant mobility and poor integration. Even in the case of successful implant integration, the patient may experience implant mobility due to chewing load;
- low aesthetics. Basal implantation does not allow achieving a result where teeth look as natural as possible: near the base of the tooth it will be visible that it is an implant.
Possible complications after basal implantation
Basal implantation is characterized by fairly low effectiveness, while there is a very high risk of complications:
- implant rejection;
- local osteomyelitis (bone inflammation);
- detachment and necrosis of the mucosa (gums) and periosteum;
- greater bone destruction;
- basal implantation of the upper jaw carries the risk of implant penetration into the maxillary sinus;
- basal implantation of the lower jaw carries the risk of inferior alveolar nerve damage.
Osadchyi Dental Clinic does not recommend the basal implantation technique to its patients. And we are not alone in the world: neither the USA nor the EU perform basal implant placement surgeries.
Strictly speaking, there are no direct medical indications for using specifically the basal dental implantation technology. Since even with a small amount of bone availability, alternative techniques with better prognosis can be applied.
At the same time, there are a number of contraindications to basal implantation:
- insufficient bone availability that prevents penetration into the basal part;
- presence of oncological diseases, diabetes, arthritis, heart disease, or mental disorders in the patient.
The price of basal dental implantation in Kyiv ranges from 30 to 60 thousand hryvnias, depending on the clinic and type of implant. The price factor of basal dental implantation is not decisive for the patient, since classical implantation has a lower cost.
The key factor for choosing this technique is the advertising promises of clinics to do everything quickly: “teeth in one day.” However, quickly does not mean well, and we especially do not advise rushing in matters concerning health.
Painful, unaesthetic, and ineffective basal dental implantation is often presented to patients with low bone availability as the only alternative. Fortunately, this is a false statement.
Osadchyi Dental Clinic specializes in prosthetics and dental implantation in Kyiv, and we always recommend proven technologies with good prognosis to our patients. We provide a lifetime warranty on our implants and promise: it will be painless, beautiful, and long-lasting.
In our work, we use plateau-system implants that can be placed even in patients with very low bone availability.
Implantation occurs in two stages:
- the implant root is placed and sutured for 45 days;
- the abutment is placed and the crown is installed.
Such two-stage implantation without immediate loading allows the implant to integrate, does not cause significant trauma to the bone, and the teeth look just like the patient’s own teeth.
The cost of plateau-system implant placement does not exceed the cost of basal implantation, yet it occurs with incomparably lower risks, faster rehabilitation, and is completely painless.
No. Our techniques allow performing implantation even with low bone availability and without bone grafting.
No, a metallic element will be visible in the gum area. It is impossible to achieve a result where basal implants look like the patient’s own teeth, even with the use of the best crowns.
The prognosis for this type of implant is poor. Even if basal implants integrate into the bone, under functional loading they become mobile fairly quickly and cannot perform the chewing function.
