A patient came to us after orthodontic treatment, referred by her treating orthodontist from another clinic. The third molars (wisdom teeth) needed to be removed to preserve the clinical results of orthodontic treatment.
The complexity of this clinical case was that the upper wisdom teeth had roots connected to the maxillary sinus, and a tooth entering the sinus cavity during extraction would be a very unpleasant complication.
The left lower tooth had roots in close anatomical proximity to the mandibular nerve, posing a high risk of neurovascular bundle injury, which could lead to temporary numbness of the lower jaw and cheek.
As evident from the description of this clinical case, extraction of such teeth must be performed with extreme caution. In such cases, we use the Yamamoto technique and extract the third molars not whole, but by sectioning them with an ultrasonic scalpel (piezotome) or bone bur.
We extracted all 4 teeth in a single visit to our clinic. The surgery took no more than 1.5 hours.
The patient’s rehabilitation period was 7 days, after which her sutures were removed.
