A patient came to us complaining of pain in the area of previously placed dental crowns, missing several teeth, tooth mobility, pus discharge and gum inflammation, requesting an assessment of her condition and treatment options. The patient provided CT scans and a report from another clinic she had previously visited.
At that other clinic, the patient was offered bone augmentation surgery and implantation using the all-on-4 or all-on-6 protocol (placement of a single bridge supported by 4 or 6 implants per jaw with screw fixation) using screw-type implants with a prosthetic structure attached afterwards. The estimated treatment time would have been 8 to 12 months.
The patient’s wish was to restore chewing function in the shortest possible time with minimal risk of complications.
We proposed an alternative treatment plan — placement of 12 short plateau-system implants, which enables complete rehabilitation of both jaws according to the patient’s wishes, with a planned treatment period of no more than 3 months.
During a single surgical procedure, the following was performed: extraction of all periodontitis-affected teeth, placement of 12 implants, bone reconstruction of both jaws, and soft tissue grafting.
The surgery lasted 2.5 hours.
The procedure was performed under local anesthesia.
The surgery involved placement of 12 short i-system implants, which allowed us to avoid bone augmentation. We used synthetic bone grafting material to fill defects (depressions at the sites of periodontitis-affected teeth), in other words, we leveled the bone contour of both jaws for a more aesthetic appearance of future implant-supported prosthetics.
The implants were placed in a pattern that keeps dental bridges separated. The future prosthetic structure will be attached to the implants using a screwless conical implant-abutment connection, which eliminates micro-mobility of prosthetic structures, thereby guaranteeing the longevity and stability of the result achieved by the patient and doctor.
The rehabilitation period will be 2.5 — 3 months. The result of the surgery is visible in the photograph.
